Does glucose influence multidien cycles of interictal and/or ictal activities?

PurposeThere are multidien patterns of seizure occurrence. Predicting risk may be easier with biomarker correlates to patterns. We hypothesize multiday hyper or hypoglycemia contributes risk.MethodsIn a type I diabetic (T1D) focal onset epilepsy continuous glucose monitoring (CGM) and responsive neurostimulation (RNS) devices, we studied interictal activities (IEA), seizures, glucose. Hourly CGM data was matched hourly RNS captures ictal over 33 months. detection settings were unchanged. Multidien cycles analyzed, active blocks IEA episodes defined, tissue averages studied.ResultsAverage 161 mg/dl. A 40-day cycle occurred, though no similar evident. Glucose elevations relative patient average associated increases in but not seizure. Frequent seizures obvious decreases from baseline, most occurred at +/- 10 mg/dl daily (i.e. 150−170 mg/dl).ConclusionTissue influence activity very frequent seizures. In an ambulatory T1D hypo hyperglycemic extremes do appear provoke activities.
